Social Security Number

You can apply for a Social Security Number (SSN) if you have received work authorization from International Student & Scholar Services for on-campus student employment or academic training. If you are applying on the basis of an on-campus job, you must first complete the J-1 Student Employment Authorization Form before you can go to a Social Security office to apply for a Social Security Number.
 

For on-campus employment, here are the steps to apply for a social security number:

  1. Fill out the application for social security card. Go to https://www.ssa.gov/forms/ss-5.pdf
  2. Call the Social Security Administration to schedule an in-person appointment. The number is 1-866-563-3450. The address is 2504 Oakmont Way, Eugene, OR 97401
  3. Bring the following documents with you to the appointment:
  • Application (filled out in blue or black ink)
  • Passport
  • Original DS-2019 (requires "wet" signature from ISSS. If you need a reprinted DS-2019 with "wet" signature, call ISSS at 541-346-3206)
  • I-94 (retrieve at cbp.gov/I94)
  • J-1 Student Employment Authorization Form (requires “wet” signature from ISSS)

You can present your receipt to your hiring department to begin being paid.

For off-campus employment, here are the steps to apply for a social security number:

  1. Fill out the application for social security card. Go to https://www.ssa.gov/forms/ss-5.pdf
  2. Call the Social Security Administration nearest you to schedule an in-person appointment. Find a Social Security Office by going to the Social Security web site and entering your ZIP code. The nearest office in Eugene is at 2504 OAKMONT WAY, EUGENE, OR.
  3. Bring the following documents with you to the appointment:
  • Application (filled out in blue or black ink)
  • Passport
  • Original DS-2019 with Academic Training Authorization (requries "wet" signature from ISSS)
  • I-94 (retrieve at cbp.gov/I94)

You can present your receipt to your hiring department to begin being paid.

 

 

NOTE: A Social Security Number is a lifetime number. If you already received a SSN in the past, you do not need to apply for a new one. Keep your SSN information private at all times. If you do not have employment, but have a taxable scholarship or fellowship that needs to be reported, you can apply for an Individual Taxpayer Identification Number (ITIN).
